Event: ONC and HHS will discuss HIT and care transitions

The Office of the National Coordinator and Health and Human Services will meet Friday for a public discussion about how health IT can improve a patient’s transition from the hospital to home. “Putting the IT in transitions” will feature talks from National Coordinator for Health IT Dr. Farzad Mostashari, HHS Chief Technology Officer Todd Park, ONC Director of the Beacon Dr. Aaron McKethan and others.

Leaving one care setting to go to another can be one of the most challenging, expensive and even dangerous parts of the health care experience, and poor handling of the transition causes millions of preventable hospital readmissions each year. This meeting will focus on near-term changes that can be made to improve care using heath IT. The ONC will also announce new policy standards for facilitating coordination.

Innovators, policy and health IT experts, health care providers, patient organizations and technology companies will be among the attendees. There are no longer spaces to attend the event on-site at The Kaiser Permanente Center for Total Health, but you can go to the event via webcast on October 14 at 10:45 a.m. EST where you can participate in an online discussion of the meeting.
